The following items are required to enroll your child at EHA (one of each per child, please):

• Application for Enrollment
• History and Physical Form
• Complete immunization record (obtain from your physician)
• If your child will be transferring from another school, please sign the Records Release Form and provide to the current school
• Emergency Information Card
• Parental Consent Form
